The lead capture form
Collect names and emails with the Form tab: choose when the form appears, make it optional, edit labels, add consent text and learn how duplicates work.
The lead capture form asks visitors for their name and email. You set it up per quiz on the Form tab. Collected leads appear on the Leads screen.
Note: A brand-new quiz has no form. Until you open the Form tab and choose a position, the form is Off and no leads are collected.
Turn the form on
- Open your quiz and go to the Form tab.
- Under Show the form, choose a position.
- Choose whether visitors can skip it.
- Adjust the fields, labels and consent text.
- Click Save, or use Next, which saves first.
Positions

If you choose Before quiz, the lead is saved before the visitor answers anything. If you choose End of quiz, the lead is saved before the result is worked out, so the lead is linked to the submission afterward.
Optional form
Under Optional form, the question “Can visitors skip the form?” has two answers:
- Yes: visitors get a Skip button and can carry on without filling anything in. No lead is saved when they skip.
- No: visitors have to fill it in to continue.
Fields
The free form has two fields.

Each field has these editable settings:
- Label
- Placeholder (the hints are “Jane Doe” and “you@example.com”)
- Required toggle (Name only, since Email is always required)
The counter at the top of the section shows how many fields are active.
Button and headings
Under Labels, set the Submit button label. The default wording depends on the position:

You can edit the title and subheading directly on the form preview.
Consent checkbox
Under Consent, the GDPR consent text box controls a consent checkbox.
- Leave it empty and no checkbox is shown on the live form.
- Type your text, for example “I agree to receive follow-up emails about my results.” and the checkbox appears.
When the checkbox is shown, visitors must tick it to continue. If they do not, they see “Please accept consent to continue.”
Whether the visitor agreed is saved with the lead and shown as GDPR consent Yes or No in the lead detail panel. It is also the consent_gdpr column in the CSV export.
Tip: Write the consent text for your own situation. Quizably only records whether the box was ticked. It does not send any marketing emails on your behalf.
Validation
The form checks the visitor’s input before sending it:
- Email: the browser checks that the field is filled in and looks like an email address. The server then checks it again and refuses anything that is not a valid email.
- Name: if you made it required, the visitor sees “Please enter your name.”
- Empty email shows “Please enter your email.”
What happens when someone submits
- The lead is saved, linked to the visitor’s quiz run.
- If you turned on the lead alert email, it is sent. See Email notifications.
- If the quiz has a webhook, a
lead_capturedmessage is queued with the lead details. See Webhooks. - The visitor continues to the next screen.
If Double opt-in is on for the quiz, steps 2 and 3 wait until the visitor clicks the link in the confirmation email. The visitor still carries on to their result. See Double opt-in.
Duplicate emails
Each quiz keeps one lead per email address. If the same email is submitted again for the same quiz, Quizably updates the existing lead instead of making a second one. The name, phone and consent value are overwritten with the newest submission, and the lead keeps the same ID.
The alert email and webhook still fire again on every submission, even for a repeat. The exception is a lead that already confirmed by double opt-in on that quiz: it is not asked to confirm again, and it is captured straight away.
The same email submitted on a different quiz is saved as a separate lead. The lead count in analytics therefore means unique emails per quiz, and leads still waiting on a double opt-in confirmation are not counted there.
If the server rejects the form
If the server refuses the submission, for example because of a network problem or because the visitor sent too many requests, the visitor sees an error message but is still allowed to continue through the quiz. In that case no lead is saved.
Visitors are limited to 60 requests a minute and 300 an hour per person, based on their address. Shared networks such as offices and schools share that allowance.
